POSITION SUMMARY:
Performs under the supervision of the Program Director/Medical Director and under the auspices of Nursing Services. Performs the functions of a Licensed Practical Nurse in assessing, planning, implementing, and evaluating assigned clients’ care in collaboration with the Clinical supervisor/RN and Physician in the clinical care. Requires a high degree of independent judgment. Plans and coordinated daily activities of the medical unit.

DESCRIPTION OF RESPONSIBILITIES:

  1. Assesses and evaluates nursing care given by self; identifies problems and need for improvement and communicates this to the Clinical Director/RN.
  2. Assesses the health status of all patients assigned to him promptly upon their arrival in the department.
  3. Assists in prioritizing the patient’s immediate needs.
  4. Completes those portions of the pertinent patient history as directed. Organizes assessment data so that it is accurate, complete and accessible. Supports and adheres to Administrative, Substance Abuse and Nursing Services Policies and procedures.
  5. Documents nursing actions implemented and effectiveness of implementation in the progress notes.
  6. In collaboration with the Clinical Director and the physician, contributes to the formulation of client care.
  7. Responsible for all charts and charting which includes chart order, reviews and other responsibilities related to charts.
  8. Documents nursing actions implemented and effectiveness of implementation in the progress notes.
  9. Explains diagnostic procedures to patient when necessary and clarifies instructions.
  10. Able to identify appropriate forms, complete forms, medical records and /or statistical record keeping as necessary.
  11. Administers medications to clients as ordered, including injections after obtaining informed consent and/or patient teaching.
  12. Upon request of the M.D. makes referrals to outside agencies.
  13. Suggests ways to solve problems and make improvements in the department.
  14. Reports abnormal diagnostic reports to the Physician.
  15. Promotes harmonious working relationships and favorable attitudes among the health care team.
  16. Knowledgeable of rules, regulations, center policies, and procedures and capable of communication the above, on the clients level, as necessary.
  17. Checks and maintains the emergency cart on the area, works with medical emergencies under the supervision of RN and/or MD, identifies and take appropriate action on medical emergencies as specified by BCLS protocols.
  18. Is knowledgeable of location, care and operation of all Emergency equipment, including but not limited to Life Pack, sterile trays, crash carts contents.
  19. Participate in conferences and meetings as necessary. Participates in any pertinent in-service education planned by the Center to improve job performance.
  20. Supports and adheres to administrative, Drug and Alcohol Program and Nursing Service policies and procedures. Ability to perform IV therapy independently utilizing universal precaution and patient safety.
  21. Evaluates self-objectives and determines need for new objectives.
  22. Evaluates the effectiveness of problems-solving techniques and activities implemented.
  23. Responsible for all charts and charting which includes chart orders, reviews and other responsibilities related to charts.
  24. Responsible for all doctor’s orders; completing lab forms, assisting the doctors with physical exams for both programs. ( Alcohol and Drug)
  25. Responsible for assigning and assisting other LPNs/aides in certain duties as specified by the Program Director.
  26. Responsible for all aides in seeing that the work is performed efficiently and effectively.
  27. Responsible for lab work and daily medication records being placed in charts.
  28. Responsible for all aides in seeing that the work is performed efficiently and effectively.
  29. Responsible for administering methadone.
  30. Responsible for checking expiration dates on all medications and supplies.
  31. Responsible for reporting to Clinical Supervisor all admissions on the day of admission.

KNOWLEDGE, SKILLS AND RESPONSIBILITIES:

  1. Has knowledge and ability to recognize medical emergencies and triage appropriately.
  2. Has knowledge of medical procedures which relate to patient education and health practices.
  3. Has the ability to carry out treatment as ordered and document appropriately within the realm of practice
  4. Provides leadership for peers and subordinate via sound nursing practice and innovative patient education
  5. Able to independently intercede as the patient’s advocate employing nursing ethics and sound nursing judgments.
  6. Able to take oral and written directions, from Program Director, physician Clinical Supervisor/RN.

MINIMUM QUALIFICATIONS:
Approved Licensed Practical Nurse course with valid Georgia license.
Experience in a health care setting and a minimum of two years supervisory experience.
Ability to communicate effectively. Bilingual a plus.

TYPICAL PHYSICAL DEMANDS:
Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye-hand coordination. Requires standing and walking for extensive periods of time. Occasionally lifts and carries items weighing up to 100 pounds. Requires corrected vision and hearing to normal range. Requires working under stressful conditions or working irregular hours. Requires exposure to communicable diseases and bodily fluids.

TYPICAL WORKING CONDITIONS:
Frequent exposure to communicable diseases, toxic substances, ionizing radiation, medicinal preparations and other conditions common to a clinic environment.
