You can call the SMC Pharmacy at 404-564-6889 or you can log into your Patient Portal and request a prescription refill from your provider.

A Patient Centered Medical Home is a model of healthcare delivery in which care is tailored to the needs and preferences of patients. The Patient Centered Medical Home model encourages active engagement of patients and families with shared decision making and practice improvement. Our goal is to provide strong provider-patient relationships, seamless care coordination, and improved access to care, all of which will result in the highest quality of medical treatment.

SMC receives a portion of its funding from the U. S. Department of Health & Human Services. Slide Fee Scale (SFS) is for uninsured and under-insured patients. SMC offers several SFS discounts of our services based on household income and family size.

I CAN NOT get through on this new phone system to make a therapy appointment. The phone system send me in a perpetual loop... If I didn't Love my therapist, Ms. YSSIS, I would b gone... 😤 😑 at the Kipp Center... AI bot says they don't have a Kipp Center... 🫩 This is Not helping my mental health!!
