Your care team includes you (the patient), your Provider, medical team members you may see during your appointments (receptionist, medical assistant or nurse), and others throughout Southside, such as Specialty providers (cardiologist, podiatrist, optometry, dental department), pharmacists, nutritionists, counselors, referral coordinators, patient navigators, care coordinators. Together, the full team provide you more support toward achieving better health outcomes.
