GENERAL DESCRIPTION OF POSITION:
This is a position reporting directly to the Lead Patient Care Coordinator under the lead of the Quality Assurance Manager. This position provides patient outreach to support care coordination efforts. Daily outreach lists and tasks will be guided by the Patient Care Coordinator. The majority of this position is calling and scheduling.
DETAILED DESCRIPTION OF POSITION DUTIES:
- Identify, call, and manage patient scheduling to meet clinical quality measures
- Manages lists or a case load of patients discerned through the Patient Centered Medical Home (PCMH) team/Care Coordination
- Tracks outreach efforts in a provided spreadsheet and the EHR system
- Generates patient lists as needed
- Provides updates during morning huddles
- Provides team support as necessary
- Other duties as assigned
